ACSS coordinates Portuguese representation in European joint action on care integration
ACSS was the chosen institution to represent Portugal in the Joint action on implementation of digitally enabled Integrated person-centred CARE-JADECARE, a project started on the 1st of October, involving fifteen European countries.
National participation is conducted by ACSS, along with SPMS – Shared Services of the Ministry of Health and the National School of Public Health.
Within the work to be carried out until 2023, like other participating countries, Portugal has chosen to implement the population stratification by risk component, which is part of the Basque Country’s ageing and chronicity strategy, considered a good practice at European level. In the future, the project will involve the participation of a restricted set of NHS´s institutions.
